New client forms and location
Please know that my practice is by appointment only
Walk-ins are not welcome
Before your first session you can fill out a New Client Intake form and General Liability Release form to help save time during your session. Please email them to MassageByJP206@gmail.com.
As of September 1st, I will be at 4137 California Ave SW in West Seattle Chiropractic Clinic. Come in and have a seat in the lobby. I will be with you shortly.
Street parking is typically available right out front of the clinic. There is parking in the back. However, you will need to walk down a few stairs to the front of the building.
If you are a new client, please complete this form and bring it along to your first session.
General Liability Release Form
This is a standard release of liability for our massage therapy work together.
