New client forms and location

Please know that my practice is by appointment only

Walk-ins are not welcome

Before your first session you can fill out a New Client Intake form and General Liability Release form to help save time during your session. Please email them to MassageByJP206@gmail.com.

I am located at 3515 SW Alaska St just off 35th on the South side of Alaska. There is parking in the back off of 36th. The back brings you directly to the second floor where the lobby is located. Please don't park in the front of the building as that is reserved for the Cat Hotel. My name is not yet on the sign so please take note of photos below so you know you are in the correct spot.

This is a standard release of liability for our massage therapy work together.
